Services to Senior Citizens
The Services to Senior Citizens Division provides services to frail adults to help maintain independent living in the least restrictive environment.
The Services to Senior Citizens Division is comprised of four programs. The programs include Adult Daily Living Center, Community Care Management, Ombudsman, and Human Services.
The program services are offered to all residents of Fayette County that meet the various program eligibility requirements.
Adult Daily Living Center
The ADLC is licensed by the Pennsylvania Department of Aging and is partially funded by the Southwestern Pennsylvania Area Agency on Aging. It provides respite for the care giver, socialization and recreation in a safe environment for the consumer.
The ADLC is dedicated to promoting independent living. Designed for persons with physical or mental impairments, the center provides a protective environment with structured activities, socialization, recreation and nutrition services.
The staff is trained in CPR, first aid, medication administration Dementia and Alzheimer’s Care. The Adult Daily Living Center is located at 100 New Salem Road, Uniontown and is open Monday through Friday from 8:00 am to 4:30 pm.
WHO IS ELIGIBLE FOR ADULT DAILY LIVING SERVICES?
- Adults, 18 and over, with a physical or mental impairment
- Residents of Fayette County
- Persons with a need for socialization
- Persons needing supervision in a structured environment

Community Care Management Program
The care management program provides assistance to Senior Citizens and their families to find resources necessary to maintain independent living.
The Care Management Program is partially funded by the Southwestern Pennsylvania
Area Agency on Aging.
WHAT IS CARE MANAGEMENT?
- Care Management is the process of providing assessment of needs and the coordination of community services to maintain independent living.
- Care Management provides follow-up on service delivery to ensure the consumer’s needs are being met.
- Care Management provides consumers with information and resources to maintain independence.
- Care Management promotes consumer choice in managing services.
WHO IS ELIGIBLE FOR CARE MANAGEMENT?
- Persons over the age of 60
- Residents of Fayette County
- Registered with the Area Agency on Aging

Ombudsman Program
WHAT IS OMBUDSMAN?
A Long Term Care Ombudsman serves as a mediator who seeks to resolve complaints made on or behalf of people 60 years of age and older who are consumers of long term care services. The Ombudsman provides a way for people to voice their concerns and have their complaints addressed. The Ombudsman tries to work with all parties to resolve a problem.
WHO USES THE OMBUDSMAN?
- Residents of Nursing and Personal Care Homes
- Individuals receiving Long-Term Care Services in their Homes and the Community
- Their Families and Friends
- Long-Term Care Facility Staff
- Government Agencies and Community Groups

Human Services Program
The Human Services Program provides support to help the person with a functional disability remain living independently in the community.

- Persons with a need for assistance to maintain their environment and remain independent
